Parent Guardian Declarations & Photo Consent
Please read below for details of Flitwick Eagles Football conditions and consent requirements that need to be signed up to, when becoming a member of the club.
Declarations (parent / guardian)

I agree that any kit supplied by Flitwick Eagles Football Club, is the property of the Club and should any item of kit issued to me become damaged or soiled (outside of Flitwick Eagles supervised activities), to the extent that it cannot be used again, or I cannot return the kit issued when asked to, I could be liable to pay for the replacement of each item.

I acknowledge and accept that Flitwick Eagles Football Club and their officers are not under liability whatsoever in respect of personal injury, loss or damage however caused whilst in attendance on any activity.

I understand that Flitwick Eagles FC will keep the registration form and all of the information supplied for the duration of the current season. At the end of the season all information provided will be destroyed in line with the Club's data protection policy.

I agree to abide by i) Flitwick Eagles Code of Conduct and the FA's Respect Programme, i) the rules and regulations of Flitwick Eagles FC, iii) the rules and conditions of use for any facility used by the club.
Photo Consent
The FA support and encourage the taking of appropriate images of children in football, with the belief that documenting a child’s involvement and progress through the season, both by film and photos, is widely accepted as contributing to the enjoyment of the game.
However, everyone wishing to film or take photos in football has a responsibility to familiarise themselves with and adhere to the FA issued guidance, which can be found here.
It’s also recognised some coaches find it helpful to use photographs or film as a coaching tool to support a player’s development.
The FA advises that coaches using filming as a legitimate coaching aid should make parents/carers and players aware that this will be part of the coaching programme and care should be taken when storing the film clips. Parental consent must of course have been given.
